Recalcitrant Wounds
78 year old male with right lateral malleolar chronic wound. Comorbidities: Type 2 Diabetes, Buergers Disease with left SYMES amputation and right Transmetarsal amputation for many years, remote tobacco use. Narcotic dependence for many years due to pain in region. Patient refusing BKA. Chronic edema on the right lower leg and varicosities. Palpable Dorsalis Pedis pulse on right. 4 years of failed treatments including: compression hose, Unna boot, Wound VAC, Profore/Oasis, Silvadene, Cytologel, Provant, Panifil, Hydrocolloid Powder, hyperbaric oxygen therapy (stopped due to decline in vision), multiple courses of p.o antibiotics and then a 6 week course of IV abx after bone scan positive for osteomyelitis.

6 Weeks Procellera™ wound dressing
Male patient received a femoral-popliteal bypass. Patient developed acute leg edema, and subsequent ultrasound exam indicated patient was positive for deep vein thrombosis. Patient was given anticoagulation meds, and option for skin grafting was eliminated. Wound VAC was applied but patient experienced adhesive allergy to the drape. Patient failed gels and other silver products.
